How do I write a police report?

Use the template below to write a police report. The opening information page with block information needs to be filled in. Next, write the narrative in the blank spaces provided. Please note that this is only an example and may not represent what other schools and departments use. 

Police Incident Report

Make sure that the report is written to the following guidelines:

  • First-person
  • Active voice
  • Chronological order
  • Past tense
  • Includes criminal elements

Narrative

  • Proper spelling/grammar
  • Short sentences / short paragraphs
  • Thorough
  • Clear, concise language (no jargon)
  • Proofread
  • Who, what, where, when, why, how

Conclusion

  • Make sure to wrap up the report at the end by tying up loose ends. Was there any evidence? What will be done with it? Was there an arrest? What is the charge against this person(s)?
