Titles should normally be in lower case within a sentence. For example: She is a registered nurse.

When using titles after or before an individual's name they are usually abbreviated. Example: Mary Smith, R.N.

Finally, some titles are registered or trademarked, so the rule is not universal. Example: He is a Realtor.
